**Please note that the application form may be completed and submitted online. This is the only form you may submit online. After submitting the application online, you need to print the last page and initial/sign it. Please send us this signature page via mail or fax as we must have a copy of your signature on file.

| Short Medical Form* | A short medical form is required for all trips except those involving high altitude or strenuous trekking. |
| Long Medical Form* | A long medical form is required for high altitude or strenuous trekking trips. |
| Insurance Waiver* | The insurance waiver is required IF you are opting out of our insurance package. |
